That's Using Your Head Sports Helmet Program

The sports helmet program is an identification and information package used on any sports helmet. Two white stickers attach to the sides of the helmet. An information label attached to the inside of the sports helmet carries important information about the child in the helmet, such as name, address, date of birth, medical history, and who to contact in an emergency.
