Case Management Services
Case Management Services offers community based support to chronically mentally ill adults or seriously emotionally disturbed or mentally ill children or youth and their families in Cortland County. These services help individuals move forward in their recovery, avoid psychiatric hospitalization, manage episodes of illness, and maintain mental health stability to the highest degree possible. By coordinating plans and resources needed to maximize successful living, case managers help consumers avoid or minimize use of high levels of care, manage crisis and plan for their long term recovery. Service coordination, advocacy and general support are part of case management process.
