In Home Education & Assessment of Asthma

Available for children between the ages of 2-17 years old living in Cortland County who have been admitted to the hospital, been seen in the emergency room or had urgent care for their asthma in the past 12 months.

The program teaches asthma self-management skills by providing education about medications, warning signs, symptoms, development of an asthma action plan with your medical provider, and home environmental assessment.
